SUMMARY OF POSITIONCase manager will be responsible to provide direct support and case coordination to the resident participating in the program with the emphasis on supporting the resident with meeting their goals. It is the case managers responsibility to maintain and update all residents daily records and /or files both in paper and/or electronically as required. Case Managers will be responsible for performing all job duties and descriptions in accordance to program policy and procedures. The Case Manager is responsible for reporting any concerns or failures to the clinician, program director or on call.JOB FUNCTIONS*Ability to work with young adults and their families in a changing setting which may include transitional care and aftercare outreach.Required to use personal vehicle, when Centerboard vehicle is not available in order to meet aftercare outreach requirements.To have a willingness to be flexible with scheduling to meet client and program needs.Demonstrate, understand and implement principals of Trauma Informed Care.Gain knowledge of community resources and work with other community agencies to coordinate services for the clients.Meet with Clinician, Program Director and Referral coordinator to discuss potential residents.Coordinate intakes as assignedIn coordination with Clinician, complete all intake paperwork within 8 hours of arriving (immediate needs assessment, releases signed, safety plan, placement agreement, mental health status, review program policies and expectations, ensure collaterals have sent any necessary paperwork).Assist and support of other youth service programs as requested.Review safety plan and mental health status form with a clinician within 8 hours of intake.Maintain all appropriate records according to collateral/Youth Service standards and guidelines.In coordination with Clinician, complete 72 hour treatment plan documents with appropriate signatures ensure youth readiness assessment YRAT is completed within 20 days of intake and signed by DCF and client. In coordination with Clinician, coordinate treatment team reviews, complete updates to treatment plans and obtain signatures. Required every 60 days after 72 hour and 30 day.In coordination with Clinician, complete CANS assessment within 30 days.In coordination with Clinician, update Safety plans every 30 days and as needed.Required to learn and use Virtual Gateway to document treatment plans, treatment plan reviews and incident reports.Support residents and staff in crisis situationsUtilize skills received in training to diffuse clients in crisis.Assist clients with coping skillsAssist co-workers with difficult clientsSupport and assist with all medical, dental and clinical appointments.Required to use language in accordance to rehab options expectations.Responsible for client binder to ensure case index and subsequent documents are signed and in order.Develop and facilitate program groups as needed to support the population.Mentor residents to increase Independent Living skills Collaborate and communicate updates and important information with DCF or other involved collaterals.Transport clients to appointments as needed, assist with providing transportation alternatives.Ensure that clients quality of life and standards are maintained including: dietary , hygiene, appropriate clothing and building cleanliness and maintenance.Assure residents quality of life standards are maintained and being met including: dietary, hygiene and appropriate building cleanlinessEnsure quality communication with the Program Director on status of clients and associated treatment plans.Provide leadership and act as a positive role model for clients and co-workers.Responsible for reading the communication log and providing client or program updates to the log as needed.On Call - Participate in the On Call rotation and document calls in Google SheetsIn coordination with Clinician, complete all Assessments/forms in accordance with State/EEC/DCF regulations- i.e. Program Assessment, YRAT, CANS, Substance Abuse Evaluation, Mental Status Exams, Treatment Plan, Biopsyc, Discharge/Termination forms, Safety Plans, and use of the Virtual Gateway.Collect Program Fees bi-weekly and submit to Fiscal- (in teen parenting programs only) and chart individual payment schedules, payments and balances.In coordination with Clinician and Program Director, conduct In- House trainings for staff- i.e Safe Sleep Tr., IR documentation refresher, BMT refresher, COVID testing, Progress and Advocate Note documentation, Staff shift responsibilities refresher and others.In coordination with Clinician, create Groups for the Program-(in teen parenting programs) mandatory to offer PAYA and parenting groups. (In all programs)- Budgeting GroupAlong with program director and clinician, plan, coordinate, and attend residential house meeting.Trainings - Keep a log of training hours and participate in trainings through Track Trainings, in person/on the phone/online. Budget Monitor resident monthly budgets and record budget information on File maker.In coordination with Clinician, PD, and Treatment Team, ensures Discharge/Transition planning for youth on caseload.Interview/Intakes with residents (adolescent programs) In coordination with Clinician, interview new residents and talk with DCF teams for possible placement. Intake new residents in the presence of the DCF SW. The case managers should present the Program and the Resident Handbook at that time and the CM will complete all intake paperwork with the youth and the DCF SW. Medication will need to have accompanied Doctors orders. The resident is expected to move in after the intake process is complete.Interview New Employee Candidates/Interns as appropriate Interview new candidates for hire and interns via FaceTime/zoom/ in person. Complete all HR interview questions. Scan and email HR background paperwork to the candidate/intern and ask them to fill them out and email them back to me along with a copy of their Drivers License. After the interview make a recommendation to accept the individual or not and email everything to the HR department for their review, or drop off the paperwork at the main office in any of the HR mailboxes. Transportation for youth under 18 years old Staff must transport and stay with a youth for the following appointments: Hospital/ER visits, collateral meetings, court, school meetings, doctor and dentist appointments.CQI, maintain, track, document, and create information necessary for the individual program. Attend meetings and present information as required. (CQI, Theory of Change )Perform any other tasks or assignments as requested.QUALIFICATIONSRequired to pass initial competency evaluation as well as continued assessments of professional development and future trainings. A Bachelors Degree is required. A combination of valid experience in social work, case management and some college/working towards a degree may be used meet degree requirements. Qualified candidates must possess strong communication and organization skills and be a willing participant in Centerboard multicultural awareness, diversity, equity and inclusion trainings, meetings and philosophy. Must have a valid drivers license and the ability to drive 12-16 passenger vans, access to and use of a personal vehicle may also be required.
