Due to business growth and demand, IQuOLIOC is seeking a Care Manager to work under the direct supervision of the Clinical Director. A Care Manager will manage and coordinate services for program participants.Responsibilities and Duties:Assesses client needs and the development of the support plan specific to the assessment outcomesManages a caseload of substantial size and complexity requiring the utilization of all community agencies serving persons with developmental disabilitiesResponsible for participant caseload, including, but not limited to annual documents, Short Range Goals, Personal Care Plans, NC SNAPs, FL-2s, and tracking of document and assessment expirationPrepares case progress notes, social summaries, and other reports concerning the participant's caseProvides staff training, remedial training, staff mentoring, and enrichmentSupervises a variety of participant care duties involving management and care of participants; assist in evaluation of staff and program needsParticipates in implementing clinical care practices; provides input on issuesParticipates in multi-disciplinary treatment planningEnsures a positive work environmentOther duties as assignedQualifications and SkillsEssential Skills Required:Knowledge of the principles of supervisionExcellent command over the language, both written and verbalWork cooperatively with othersEducational/Professional Qualifications:Masters degree in human service field and a minimum of one year full-time, post-graduate degree accumulated IDD experience with the population served preferred. Bachelor's degree in a human service field and two years of full-time, post-bachelor's degree accumulated IDD experience with the population served or bachelor's degree in a field other than human services and four years of full-time, post-bachelor's degree accumulated IDD experience with the population served minimally required.
